Tonsils (palatine tonsils)  are small, oval-shaped glands located at the back of the throat, which form part of the body’s immune system. They help trap and fight bacteria and viruses that enter through the mouth and nose, but can sometimes become inflamed or infected, leading to tonsillitis. While more common in children, tonsillitis can impact adults, causing persistent throat pain, difficulty swallowing and recurrent infections.

A GP would usually suggest that a patient sees a specialist ENT doctor if they are affected by tonsilitis more than four times a year. However, I would suggest that if a person is only affected 2-3 times each year but this has occurred over 2 consecutive years, that it is important to see a specialist ear, nose and throat doctor who can investigate the underlying causes and delivering lasting solutions.

Diagnosis

Accurately diagnosing tonsillitis is pretty straightforward, but an ENT doctor can assess you more thoroughly with a focus to understanding the root cause of recurrent tonsil infections:

  • Medical History: Symptoms often include severe sore throats, swollen glands, difficulty swallowing and fatigue. An in-depth discussion about your health, might indicate that reflux or allergies are playing a role in your condition
  • Physical Examination and Endoscopy: Some patients experience recurrent infections and will have taken several courses of antibiotics already. It is easy to see white patches on their tonsils (which suggests bacterial involvement) when looking in a person’s mouth. In my office I will also look beyond the back of the mouth and down the throat with an endoscope to determine if something beyond the tonsils is contributing to issues
  • Further Investigations: Throat swabs or blood tests may be organised to identify the bacteria or assess any immune system concerns

Treatment Options

Tonsillitis can be frustrating, particularly when it becomes recurrent or increasingly severe.  My approach is to ensure patient’s receive clear answers and guidance on the different options available. The treatment plan is always tailored to each patient’s needs, depending on the severity of symptoms and the frequency of infections. Treatments include:

  • Medication: If bacterial tonsillitis is confirmed, a targeted course of antibiotics can help eliminate the infection. However most patients have tried many different antibiotics before seeking help of a specialist ENT doctor
  • Tonsillectomy (Surgical Removal): For those experiencing repeated or prolonged episodes, surgical removal of the tonsils may be the best course of action. This procedure provides long-term relief, preventing future infections and improving overall throat health.
  • Lifestyle: In some cases, avoiding irritants like smoking or alcohol, or tackling underlying allergies, is enough to prevent recurring tonsilitis

Repeated courses of antibiotics for tonsillitis can become less effective over time and may contribute to antibiotic resistance or other health concerns.
